ESR Alumni/ae Survey
The online form of the ESR Alumni/ae Survey is in PDF format (271KB). The PDF document opens in Acrobat Reader (you will need to have it installed on your computer). Enter your information in the fields provided. You will be able to tab from field to field. As a text box fills, the font size adjusts to allow you to enter more data. Click on the check boxes to indicate your choice. When you have finished taking the survey, please print a copy for your records (you will not be able to save the file with Acrobat Reader). At the end of the survey is a submit button. When you click that button, it will enable you to email the data to us (the filename of the attachment will be alumsurvey.xfdf). If you use a web-based email program, Acrobat Reader will provide on screen instructions for how to email the attachment.
- If you have trouble emailing the file and have printed your survey, please just mail us a copy of your survey: Earlham School of Religion, 228 College Ave., Richmond, IN 47374.
- If you aren't sure how to fill out the form online in Acrobat Reader but are able to open the document, you could simply print out the form, fill it out by pen, and then mail it to the above address.
- If you don't have Acrobat Reader, but want to fill out the form online, here is the link to download and install Acrobat Reader. It is a 27MB file.
- If you don't want to fill out the form online and do not have a copy of the survey from a mailing, please contact ESR and we will mail a form to you.
We hope to have participation from as many alumni as possible, so that our records can be updated and complete. Updating your information will improve our ability to keep you in the loop about what's happening at ESR and among alumni, help us update the on-line alumni directory, and tell us about what you have been up to since graduation.
Because this is the first comprehensive alumni survey in many years, your participation is important. Please help ESR by taking part in this important work.
